ABSTRACT: Nearly ninety percent of the 8 million individuals with Parkinson disease (PD) worldwide have significant speech deficits that negatively impact their quality of life. Over the past twenty years, an efficacious speech treatment for PD (LSVT LOUD) has been developed with outcome data supporting long lasting improvements (out to 2 years) that are correlated with brain reorganization as revealed by recent neural imaging studies. This treatment is organized around a simple but powerful therapeutic principal: to increase vocal loudness (targeting amplitude of respiratory-laryngeal movement) in individuals with PD while re-training the sensory motor processes involved in disordered speech communication. Treatment effects extend beyond the respiratory-laryngeal focus to include improvements in articulation, facial expression, swallowing and communicative gestures. These positive treatment outcomes dove-tail with recent findings in animal models of Parkinson disease that reveal exercise favorably impacts behavioral characteristics of the disease as well as its neural substrates. Many of the key elements of exercise that promote disease modification in animal models (intensive training, repetition of motor tasks, complexity of tasks, and active engagement) are incorporated into the LSVT protocols. This presentation will: 1) present the background and motivation for voice treatment in Parkinson disease, 2) describe the creation of an efficacious treatment and efficacy data, 3) report on unexpected outcomes and discoveries, 4) discuss application of principles of LSVTR to intensive whole body amplitude-based training protocol for individuals with Parkinson disease (LSVTR BIG; LSVTR HYBRID), and 5) demonstrate innovative technology (e.g., Virtual therapy) to enhance accessibility of treatment for all people with PD who can benefit.
